A completed form with full payment must be received on or before the specified cut-off dates as reference above. Payments received after that date will not be honored. There are no refunds or credits issues for no shows or cancellations. No exceptions. You may change the name on your registration/badge no later than 5/6/17 at 5pm. No extension.
